People with TB who do not respond to at least isoniazid and rifampicin are said to have MDR-TB. People who are resistant to isoniazid and rifampicin,plus any fluoroquinolone and at least one of the three injectable second-line drugs(Amikacin, kanamycin, or capreomycin) are said to have XDR-TB.

To treat drug resistant tuberculosis, a new drug, Pretomanid is approved by U.S. food and drug administration (FDA).

It was developed and tested by the New York-based NGO TB alliance. NPO has granted a license to Mylan to manufacture and commercialise it.

What is new ?

It will reduce the time of treatment to drug resistant TB to 6 months when used with bedaquiline and linezolid. Having 91% success rate which will reduce the death from the TB.

The three drug regimen is meant only for treating pulmonary TB and should not be used for treating extra-pulmonary TB,drug-sensitive or latent TB.
The safety and effectiveness of the drug has not been tested in pregant women and children.

Adverse drug reactions caused by the drug ?

It has caused serious adverse reactions including liver toxicity (hepatotoxicity), suppression of bone marrow activity leading to reduced production of RBC, WBC and platelets as well as damage to peripheral and optic nerves.

Lactate accumulation in body leading to sharp reduction in the pH of the blood. Possibility of an abnormal and potentially fatal heart rhythm.

Other adverse reactions seen were peripheral neuropathy, anaemia, indigestion, visual impairment, abnormal loss of weight and diarrhoea etc.

Concluding remark : According to WHO's Global Tuberculosis report 2018, an estimated 4.5 lakh people across the world have MDR-TB and nearly 37,500 people have XDR-TB. India has 24% of MDR-TB cases in the world. So the availability of a potent drug is welcome news.Making drug affordable will be highly commendable and a desperately needed model to be followed.
